An important reminder for anyone getting temporary housing assistance from the Federal Emergency Management Agency: the agency said it mailed a form to applicants about two weeks after they got their rental grant, and anyone who wants to remain eligible needs to fill out the form and send it back.

The form is titled "Declaration for Continuing Need for Rental Assistance."

Anyone who doesn't have the form should call FEMA's toll-free help line at 800-621-3362 or go online to disasterassistance.gov.
